Riva partners with clinical groups in both FFS and VBC capacities. We select potential clinical partners who are well aligned and interested in widespread participation for their eligible patient populations. We then create custom partnership plans with these groups. Depending on your practice, Riva will net out to either provide increased revenues in a FFS capacity or ROI positive cost savings for VBC partners. Riva clinicians include fully licensed cardiologists / APPs, NPs, and PharmDs, as well as certified health coaches. Riva clinicians have training and experience from some of the nation’s leading medical institutions, including Johns Hopkins, UCSF, National Jewish Health, and more. Riva provides full clinical management according to JNC8, ACC-AHA, ESC/ESH, and KDIGO standards.
Riva is designed to be flexible and lightweight. After signed agreement, Riva can be made available to folks as soon as the next day. However, we have seen most success working with groups to develop communication plans that properly announce and inform individuals of Riva’s availability a few weeks prior to launch.
We do not replace an individual’s primary care provider or cardiologist, rather we compliment the work that they are doing. Once a month, we provide the patient with a print out that they can share with any doctors they are working with to show them the progress they are making with Riva and ensure that their care across providers is cohesive. Their doctor can also access this information directly from the health records Exchange.
If you determine that Riva isn’t properly addressing the needs of your population, no worries! Riva will offboard all enrolled patients and ensure their care plan is up to date and available for the patient’s doctor. If the individual decides they want to continue with Riva on their own, there are out of pocket plans available for continued care.
Riva Health is an exclusive program designed to help individuals with hypertension and elevated CVD risk get and keep their blood pressure under control. General eligibility criteria for Riva includes, but is not limited to:
- Diagnosed hypertension or BP higher than 140/90
- Resident of CO, UT, or FL
- Is not pregnant or on dialysis
May have one or more of the following:
- Elevated BMI
- Hyperlipidemia or dyslipidemia
- Coronary Artery Disease
- History of heart attack or stroke
- Elevated A1C, known diabetes or pre-diabetes
- Elevated ASCVD risk
- Age over 55
