No, Riva is designed for time saving and interoperability. Simply prescribe Riva to your patients with hypertension or elevated CVD risk rather than a medication. Riva manages your patients in between visits, providing you with monthly summary updates to your EHR and escalations when necessary. Riva updates patient information via Athena, which can be easily accessed through the Exchange. Riva also participates in groups committed to interoperable solutions like Commonwell and Care Everywhere.
Riva Health is an exclusive program designed to help individuals with hypertension and elevated CVD risk get and keep their blood pressure under control. General eligibility criteria for Riva includes, but is not limited to:
- Diagnosed hypertension or BP higher than 140/90
- Resident of CO, UT, or FL
- Is not pregnant or on dialysis
May have one or more of the following:
- Elevated BMI
- Hyperlipidemia or dyslipidemia
- Coronary Artery Disease
- History of heart attack or stroke
- Elevated A1C, known diabetes or pre-diabetes
- Elevated ASCVD risk
- Age over 55
